                  Please Note: This work will probably have to be approved by the LOC committee.


           

           Please note: This form is for requesting the alteration of existing facilities. 
                 Things like:  a wall to be moved or removed, a new wall outlet added, 
                 a light switch moved, new carpet, walls painted, etc, etc,  
  
                 If you found a carpet stain, leaky faucet, light bulbs needing replaced, etc
